The article presents the results of treatment of 173 patients with postcholecystectomy syndrome and organic changes in the sphincter of Oddi and 140 patients with functional changes in the sphincter of Oddi. The main direction of therapeutic management for organic causes of postcholecystectomy syndrome was maximum use of endoscopic methods and endosurgery in combination with modern therapeutic agents. When the functional changes in the sphincter of Oddi were detected, drug therapy was applied with a differentiated approach to the choice of drugs, depending on the prevalence of hypertonia or sphincter of Oddi dysfunction (mebeverine hydrochloride (duspatalin), itopride, trimebutine maleate (trimspa). For normalization of the physicochemical properties of bile and prevention of recurrent choledocholithiasis, ursodeoxycholic acid derivatives (Ursofalk) were used.
