Category of quality in health care: transformation definition

Background. The purpose of the research: to define the content of the concept of quality at the present stage of development of the world, in particular, the domestic sphere of health care. Objectives of the study: analysis and generalization of the results of scientific and international researches on the topic in a chronological aspect. Materials and methods. Scientific literary sources (7 units), international documents (11 units), normative legal acts (3 units) were reviewed. We used such methods as system approach, system and content analysis, graphical. Results. The transformation of the concept of quality in health care was traced from 1860, when the axiom “First of all, do no harm” was the main characteristic of quality, until 2018, when clinical and economic efficiency, patient focus, equity, safety, accessibility (comprehensive access coverage) were attributed to the quality characteristics. The transformation of the concept in the chronological aspect took place depending on the views of scientists (Thomas Sidengam, A. Donabedian, John Ovretveit), and then experts - representatives of WHO, World Bank, OECD. It was found that the negation of preliminary quality characteristics in the updated definitions did not occur; over time, the characteristics were supplemented and acquired a deeper content in accordance with the changing needs of stakeholders in the quality of the parties: patients, professionals, health care managers. A significant role in the implementation of the quality phenomenon in health care now belongs to primary health care. Conclusions. Changes in the concept of quality in the health sector can be used in the revision and optimization of the quality policy and strategy by the leadership of the domestic industry and health care institutions, as well as local territorial communities in planning activities to improve the quality of medical care and improve public health.
