Нейропсихологічні механізми суїцидальної поведінки

У статті представлено сучасне розуміння нейропсихологічних механізмів формування суїцидальної поведінки. Описано види суїцидальної поведінки та дано їй інтерпретацію з позицій аксіології. Представлено епідеміологію суїцидальної поведінки з урахуванням вікових, гендерних, професійних та соціальних характеристик. Згруповано найважливіші екстраперсональні та інтраперсональні чинники суїцидального ризику. Порівняно механізми виникнення суїцидальної поведінки у психічно хворих (шизофренія, маячні розлади) та психічно здорових осіб. Висвітлено зв’язок між депресією та виникненням суїциду. Представлено причини доведення до самогубства, у тому числі розглянуто медпрепарати з депресогенним ефектом. Розкрито нейропсихологічний аспект психодинаміки суїцидальної поведінки. Показано нейрофізіологічну основу суїцидального мозку, яка проявляється дисфункцією вентральної, гіперфункцією дорзолатеральної і гіпофункцією орбітофронтальної частин перфронтальної кори; гіперактивністю неокортексу правої півкулі мозку та зменшенні правого парагіпокампа; зміною активності блукаючого нерва. Висвітлено нейрохімічні зміни в суїцидальному мозку, що пов’язані зі зниженням серотонінергічної активності загалом та більш високою активністю серотонінових рецепторів у лівій орбітальній корі. Описано психологічні особливості суїцидального мозку, які проявляються відчуженим ставленням до свого тіла, підвищенням больового порогу, ослабленням когнітивних функцій (візуальної і вербальної пам’яті, порушенням зорово-просторової орієнтації, ослабленням оцінювальної функції, порушенням уваги, ослабленням оперативної пам’яті, нездатністю розпізнавати небезпечні ситуації і користатися своїм життєвим досвідом), збереженням інтелекту та цілеспрямованістю у поведінці, депресією, нездатністю подолати стрес.References
